The Agility Challenge Acres Club

Agreement: In applying for Club Membership & Training Privileges, I am stating that I intend to further train with my dog(s) in Agility, and that I will fulfill the following requirements within one (1) year from the date of this application.
1. Volunteer at least 25 hours completing items on the CLEANING CHECKLIST form, in accordance with CLEANING CHECKLIST guidelines
2. Volunteer at least 3 hours at competitions/events held at the facility (UKI/ISC/seminars)
3. Take at least 1 hour of lessons with Daisy Peel each month

I understand that THE AGILITY CHALLENGE is a private club, and that policies regarding use of the facility is set by the owners and operators, Daisy Peel, George Mariakis, and Courtney Farmer. As a member of THE AGILITY CHALLENGE club, it is my duty to share in the responsibility and workload of the club and contribute to its success. I understand and agree to facility policies and turf use rules and understand that my membership can be revoked at any time for violating the policies, with no reimbursement of any membership fees paid. I also understand and agree that I will not use the facility for anything other than dog training, nor shall I accept payment of any kind for giving dog training lessons at The Agility Challenge Arena unless under a specific contract or rental agreement. I agree that I will not share my rental time or training privileges with anybody who is not also a member with training privileges.

I have read and understand the above. I agree to make every effort to meet the requirements within one (1) year from the date of this application. If I fail to complete the requirements within one (1) year, I acknowledge that I will be required to re-apply for Training Privileges after a six (6) month waiting period should I still wish to pursue club membership. I also agree to the GUIDELINES FOR MEMBERS’ CONDUCT and MEMBERSHIP AND POLICIES documents that follow this page.

I have attached an application fee of $25.00 with this completed application.
